Waivers and Service Agreements

|Individual COVID waiver||For reference. This form is filed from your account dashboard.|

This will be provided at your event by EBRPD Staff. It is designed for walkup volunteers.
|Please fill this form out for Youth under the age of 18. It must be signed by a parent or legal guardian. Youth under the age of 13 must be accompanied by an adult.|
|Please bring this form if you are a teacher or organizer bringing a group of youth to a volunteer event.|
